One hazard, scored twice
Every hazard carries an initial score and a residual score. The distance between them is the argument a safety case makes — and it is invisible from one number, which is why a hazard log with a single column proves nothing.
Note the severity did not change. Controls usually change how often something goes wrong, not how bad it is when it does — recording a reduced severity after controls is one of the commonest mistakes in a hazard log.
